Medication Administration
If at all possible, it is best to take medication at home.
To have medication administered at school, the following must occur:
- The treating physician must fill out the School Medication Administration Form. This must be done for both prescription and non-prescription medications. Clear instructions regarding the name of medication, dosage, time of day and how often it should be given must be included on the authorization form.
- Parent/Guardian must sign the bottom section of the authorization form.
- Medication must be sent to school in pharmacy-labeled bottles with the appropriate dispensing information. Over-the-counter medication must be in its original labeled container with the ingredients listed. The student’s name must be affixed to the container.
- A new medication authorization form must be submitted with each new school year, and any time there is a change in dosage or a change of medication.
